We also have Sliding Scale payment options available for those who qualify.

If you fail to come to two scheduled intakes or fail to provide a minimum of 24 hours notice that you will be unable to come to your intake appointment then The Telos Project will be unable to offer you services.

Your First Appointment

Please contact our front office to schedule your initial appointment. Once your initial appointment has been scheduled we will email you our intake forms directly. We ask that you please arrive at least 30 minutes early to your first appointment and have all of your intake forms completed and brought with you. 

Divorce/Adoption/Foster Parent/Guardianship

If the patient is under the age of 18 and there is a situation such as divorce, legal separation, guardianship, foster care, or adoption is involved please bring all documentation to your first appointment.

Required Documentation

If you are interested in psychological, neuropsychological, and/or psychoeducational assessment we kindly request you bring ALL of the following with you to the first intake assessment or fax ALL of the following documentation to our office PRIOR TO THE INITIAL INTAKE ASSESSMENT:

  1. A referral for psychological assessment from the patient’s primary care physician (this can be a general practitioner, family doctor, and/or pediatrician).

  2. Any clinical notes and/or documentation stating problems the patient may be experiencing in their associated environments (i.e. home, school, work) and/or behavioral observations from someone besides the patient (i.e. a parent, guardian, spouse, partner, therapist, counselor, teacher, mentor, etc.)

  3. Any documentation (written on company letterhead) from schools, day cares, etc. describing problems and their coinciding behaviors.

Failure to provide the above documentation in a timely manner may prevent The Telos Project from moving forward with your requested assessment.

Late Arrival Policies

Therapy

Your therapy appointment time(s) are set-aside specifically for you. The session will need to begin and end on time. If you are more than 20 minutes late to your appointment, you will not be seen, and will be charged a missed therapy appointment fee of $50, which will be automatically charged to your credit card on file.

If you need to cancel your appointment, please provide at least 24 hours notice. If you cancel your appointment with less than 24 hours notice you will be charged a late cancellation fee of $50.

If you fail to show up for your appointment you will be charged a no show fee of $50.

People who are using Medicaid and/or EAP will not be charged a late cancellation and/or no show fee.

Assessments

You are required to provide at least 72 hours notice if you are unable to make your assessment appointment or you will automatically be charged a missed psychological assessment fee of $150 to the credit card you provided.

Patients seeking services through the umbrella of EAP or who are utilizing Medicaid are not subject to no show/late cancellation fees.

Sick Policy

If you feel ill, we kindly request you stay home to recover and see your primary care physician as soon as possible.

Missed appointments for illness canceled after the 24 hour period will not be charged a missed appointment fee. We will attempt to reschedule your appointment at our earliest convenience.

Sliding Scale

You may qualify for our sliding scale rates if you are a family of 2 or fewer and make less than $50,000 annually OR if you are a family of 4 or more and make less than $75,000. We need proof such as a pay-stub to qualify our sliding pay scale.
